Sharing your medical record
Your medical record is your property and can only be opened and transferred to another doctor with your permission. If you choose to keep doctor Heino Folmer as your regular family doctor in Portugal or if you want to register as a new patient, you can request your medical file from your previous practice, preferably digitally or on paper. You can then hand this over to the assistant during your next consultation, or upload it via the button / link below.
Sharing other medical information
If you would like to share important medical information (e.g. results) from other practices or clinics with Dr Folmer, please hand it over to the doctor at your next consultation, or upload it using the form below.
We consider your privacy to be of the utmost importance, so the above data will be sent in a fully secure way and will only be seen by the assistant(s) and the doctor.
